 |
| It was vaccinated after it was fitted with a microchip |

| The vet has correctly filled in the rabies details in the EU pet passport |
 |
| The vet has signed this off with a stamp and a signature of practice |
 |
| 21 days have passed since it was vaccinated against rabies |
 |
| The vet treated my dog for tapeworm 1-5 days before it will arrive in the UK |
 |
| The vet has correctly filled in sections IV, VI and VII of the EU pet passport |
 |

* If your pet was successfully blood tested and issued with an EU pet passport before it left the UK or other EU country to go to a non-approved country, the 3 months waiting period before re-entering the UK will not apply (but all other rules still apply, including the requirement for the blood sample to be taken at least 30 days after vaccination |
